Homelessness represents a significant social and public health challenge in California. Illumination Foundation (IF) is an expanding non-profit organization committed to breaking the cycle of homelessness by offering specialized, interdisciplinary services within our recuperative care facilities, emergency shelters, housing initiatives, and programs for children and families. IF currently operates over 13 facilities with more than 22 micro-communities throughout Orange County, Los Angeles County, and the Inland Empire.

Position Overview

The Medical Coordinator (LVN) plays a crucial role in providing exceptional medical care and fostering positive health outcomes for individuals experiencing homelessness in a recuperative care environment.

The compensation for this role is $34.00 per hour, based on experience. Additional compensation includes a $1.50 swing shift differential and a $3 graveyard shift differential. Standard working hours are from 7:00 AM to 3:30 PM.

Key Responsibilities

Client Care:
  • Conduct initial assessments to establish an appropriate care plan based on discharge information from hospitals or primary care providers.
    • Evaluate the health status of patients upon arrival through comprehensive assessments and vital checks.
    • Determine insurance eligibility, schedule follow-up appointments, and review medication lists.
  • Develop and maintain individualized care plans.
    • Set and monitor weekly goals to assist patients in medical or social reintegration, verifying completion and assessing ongoing medical needs.
  • Perform regular health checks throughout the day to monitor the well-being of assigned clients, ensuring no changes in medical conditions since discharge.
  • Facilitate access to medical services as needed (insurance, primary care providers, specialists, referrals).
  • Coordinate home health visits and follow up on medical appointments as necessary.
  • Enhance health literacy by providing education on illnesses, medications, preventive measures, treatment management, and overall health.
  • Arrange transportation for clients to and from medical appointments when required.
  • Report emergencies to the Supervisor/Manager and document incidents.
    • Seek appropriate care for medical complications.
    • Initiate medical response protocols when necessary.
  • Support patient recovery by providing essential items and a conducive environment for rehabilitation while assisting with facility maintenance.
Administrative Duties:
  • Input patient data and case notes into electronic health records and other documentation systems.
  • Update medication records, vital signs, progress notes, and care plans.
  • Handle administrative tasks such as answering calls and organizing paperwork.
  • Prepare patient reports for billing purposes.
  • Maintain an adequate inventory of medical supplies and medications.
Compliance and Safety:
  • Ensure confidentiality in line with HIPAA regulations, Illumination Foundation policies, and Department of Health Services standards.
  • Provide patients with necessary compliance forms upon admission.
  • Implement infection control protocols in accordance with OSHA standards.
  • Conduct medication reconciliation and ensure proper disposal of biomedical and pharmaceutical waste.
  • Maintain a safe working environment by ensuring cleanliness in medical and medication storage areas.
Qualifications

Required:
  • Current and unrestricted LVN license in California.
  • Minimum of 2 years of relevant experience.
  • High School Diploma or equivalent.
  • Basic computer proficiency, including email communication and data summarization in spreadsheets.
  • Valid California Driver's License.
Preferred:
  • Experience in home health nursing and working with populations facing addiction and behavioral health challenges.
  • Bilingual proficiency in English and Spanish.
  • Familiarity with Microsoft Office Suite (Outlook, Word, Excel, Teams) and electronic health record systems.
