Position Type: Per Diem - Float

Compensation: $29.85 / Hourly (Compensation may vary based on relevant experience for the role.)

Role Overview:

The Licensed Practical Nurse Clinical Associate will support the Resident Care Director in overseeing the Resident Care Aides, assisting with medication management, daily living activities, and monitoring the medical and clinical needs of residents.

Key Responsibilities:
  • Monitor residents' physical, emotional, social, and cognitive well-being.
  • Collaborate with the resident care coordinator to facilitate transfers to different care levels as necessary.
  • Keep physicians and families informed about changes in resident conditions and ensure adherence to medical directives.
  • Assist in evaluating the functional status of applicants as required.
  • Support the resident care coordinator in reviewing all medical documentation.
  • Ensure completion of admission forms prior to resident intake.
  • Assess each resident's health and functional capabilities.
  • Communicate health status updates to residents and their families.
  • Coordinate care needs and medical orders with residents' primary physicians.
  • Maintain accurate nursing records for all residents.
  • Uphold resident confidentiality at all times.
  • Direct resident care in the absence of the resident care coordinator.
  • Ensure adequate staffing coverage consistently.
  • Assist in establishing medication assistance programs for residents requiring such services.
  • Supervise resident care assistants in maintaining a safe medication administration process.
  • Take responsibility for the proper storage of medications not kept by residents.
  • Maintain medication records, review and transcribe medication orders, and ensure all orders are properly signed.
  • Ensure compliance with medication administration records (MAR) and that resident care assistants adhere to MAR instructions.
  • Keep the resident care coordinator informed of any medication changes.
  • Facilitate refill requests for residents on medication assistance programs and notify families when necessary.
  • Provide education to residents regarding medications and medical equipment as needed.
  • Communicate relevant information to other staff members.
  • May be assigned on-call duties on a rotational basis to address resident and program emergencies.
  • Required to work every other weekend.
  • Conduct training sessions for resident care assistants as needed.
  • Assist the resident care coordinator with training initiatives.
  • Ensure adherence to policies and procedures by resident care assistants.
  • Participate in quality assurance activities.
  • Conduct audits related to the nursing program as necessary.
Qualifications:
  • Strong nursing assessment skills.
  • Collaborative team approach in working with staff.
  • Ability to interact courteously and tactfully with staff, residents, families, visitors, vendors, and the public.
  • Current licensure and registration as a licensed practical nurse.
  • Three years of experience working with older adults.
  • Licensure and registration as a registered nurse may substitute for the LPN requirement and part of the experience requirement.

Safety Responsibilities: Employees must be fully acquainted with all safety policies and procedures. Active participation in fire drills and emergency procedures is required. Compliance with facility in-service requirements is mandatory. Confidentiality of all information related to the organization, residents, families, and staff must be maintained at all times.
