Position Type: Per Diem - Float

Compensation: $29.85 / Hourly (Compensation may vary based on relevant experience for the role.) and additional shifts available.

Role Overview: LPN Clinical Associate

The LPN Clinical Associate will support the Resident Care Director in overseeing the Resident Care Aides, assisting with medication management, daily living activities, and evaluating the medical and clinical needs of residents.

KEY RESPONSIBILITIES
  • Monitor residents' physical, emotional, social, and cognitive well-being.
  • Collaborate with the resident care coordinator to facilitate transfers to different care levels as necessary.
  • Keep physicians and families informed about changes in resident conditions and ensure adherence to physician care instructions.
  • Assist the resident care coordinator in evaluating applicants' functional capabilities as required.
  • Aid in reviewing all medical documentation with the resident care coordinator.
  • Ensure completion of admission DSS 3122 prior to resident entry.
  • Assess the health and functional abilities of each resident.
  • Communicate health status updates to residents and their families.
  • Collaborate with primary physicians to coordinate care requirements and medical directives.
  • Maintain accurate nursing records for all residents as mandated.
  • Uphold resident confidentiality at all times.
  • Direct and assign resident care in the absence of the resident care coordinator.
  • Ensure adequate staffing coverage at all times.
  • Assist the resident care coordinator in establishing a medication assistance/reminder program for residents requiring such services.
  • Supervise resident care assistants in implementing a safe medication assistance program.
  • Take responsibility for the proper storage of all medications not retained by residents.
  • Maintain medication records, review and transcribe medication orders, including new prescriptions and changes, and ensure all orders are duly signed.
  • Ensure compliance of resident care assistants with MAR instructions and that they sign MAR in accordance with health department regulations.
  • Keep the resident care coordinator informed of any medication changes as necessary.
  • Manage refill requests for residents on the medication assistance program and notify families when they pick up refills.
  • Provide education to residents regarding medications and medical equipment as needed.
  • Relay all relevant information to other staff members.
  • May be assigned on-call duties on a rotational basis to address resident and program emergencies as required.
  • Required to work every other weekend.
  • Conduct in-service training for resident care assistants as necessary.
  • Assist the resident care coordinator with in-service training as needed.
  • Ensure adherence to policies and procedures by resident care assistants.
  • Participate in quality assurance initiatives.
  • Conduct audits related to the nursing program as required.
QUALIFICATIONS:
  • Strong nursing assessment capabilities.
  • Collaborative team approach with other staff members.
  • Ability to interact courteously and tactfully with staff, residents, family members, visitors, vendors, and the general public.
  • Current license and registration as a licensed practical nurse.
  • Minimum of three years' experience working with older adults.
  • Registration as a registered nurse may substitute for the LPN requirement and part of the experience requirement.

Duties of this role require occasional walking and standing in and around the reception area and lobby. Must be able to occasionally lift loads of 50 pounds without assistance, and the ability to sit, talk, and hear is essential. SUPERVISORY RELATIONSHIP: Work is performed within established guidelines and requires periodic supervision. The employee exercises some initiative and independent judgment to effectively perform the essential job duties of this position. RESPONSIBILITY FOR OTHERS: Employee assists in supervising resident care staff.

SAFETY RESPONSIBILITIES: Must be fully acquainted with all safety policies and procedures of Episcopal Church Home. Takes an active role in all fire drills during shifts and/or emergencies for internal and external disasters as monitored by supervisors. Must comply with facility in-service requirements. Maintain confidentiality of all information related to the organization, residents, participants, family, and staff that may be encountered during the normal course of business. This includes medical and treatment records, financial, and human resources information.
