Position Type: Per Diem - Float
Compensation: $29.85 / Hourly (Compensation may vary based on relevant experience.)
Role Overview:
The Licensed Practical Nurse Clinical Associate will collaborate with the Resident Care Director to oversee the Resident Care Aides, assist with medication management, support daily living activities, and assess the medical and clinical requirements of residents.
Key Responsibilities:
- Monitor residents' physical, emotional, social, and cognitive well-being.
- Coordinate with the resident care team to facilitate transfers to appropriate care levels when necessary.
- Keep healthcare providers and families informed about changes in resident conditions and ensure adherence to medical directives.
- Assist in evaluating applicants' functional capabilities as needed.
- Review all medical documentation with the resident care team.
- Ensure completion of admission documentation prior to resident intake.
- Evaluate each resident's health status and functional abilities.
- Communicate health updates to residents and their families.
- Collaborate with primary care physicians to align care needs and medical orders.
- Maintain accurate nursing records for each resident.
- Uphold resident confidentiality at all times.
- Direct resident care in the absence of the resident care coordinator.
- Ensure consistent staffing coverage.
- Assist in establishing medication assistance programs for residents requiring such services.
- Supervise care assistants in executing a safe medication assistance protocol.
- Manage the secure storage of medications not retained by residents.
- Maintain and review medication records, including new prescriptions and changes, and ensure compliance with health regulations.
- Keep the resident care coordinator informed of any medication updates.
- Facilitate refill requests for residents on medication assistance programs and notify families of pickups.
- Provide education to residents regarding medications and medical equipment as necessary.
- Share relevant information with other staff members.
- May be required to participate in on-call duties for emergencies.
- Work every other weekend as part of the schedule.
- Conduct training sessions for resident care assistants as needed.
- Support the resident care coordinator in training initiatives.
- Ensure adherence to policies and procedures by resident care assistants.
- Engage in quality assurance activities.
- Conduct audits related to nursing programs as required.
- Strong nursing assessment capabilities.
- Collaborative approach to teamwork.
- Ability to interact respectfully and tactfully with staff, residents, families, visitors, and vendors.
- Current licensure and registration as a licensed practical nurse.
- Minimum of three years of experience working with older adults.
- Registered nurse licensure may substitute for LPN requirements and part of the experience criteria.

Supervisory Relationship: Work is conducted within established guidelines and requires periodic supervision. The employee exercises initiative and independent judgment to effectively fulfill the essential job duties of this position.
Responsibility for Others: The employee assists in supervising resident care staff.
Safety Responsibilities: Must be familiar with all safety policies and procedures. Active participation in fire drills and emergency preparedness is required. Compliance with facility in-service requirements is mandatory. Maintain confidentiality of all information related to the organization, residents, families, and staff encountered during the course of business.
