We are seeking a highly skilled and experienced Care Manager to join our team at Direct Recruit Agency. As a leading recruitment agency in the healthcare industry, we are committed to providing high-quality care to our clients.
Key Responsibilities:- Oversee the day-to-day operations of our care services, ensuring the delivery of high-quality care to our clients.
- Manage a team of care staff, providing guidance, support, and training as needed.
- Develop and implement care plans for clients, ensuring they are tailored to their individual needs and regularly reviewed and updated.
- Conduct regular assessments of clients' care needs and make necessary adjustments to their care plans.
- Maintain accurate and up-to-date records of clients' care and progress.
- Liaise with clients, their families, and healthcare professionals to ensure a coordinated approach to care.
- Monitor and manage the budget for care services, ensuring efficient use of resources.
- Stay up-to-date with industry developments and best practices, implementing changes as needed to improve the quality of care provided.
- Adhere to all relevant policies, procedures, and regulations to ensure compliance with legal and ethical standards.
- Handle any complaints or concerns from clients or their families in a professional and timely manner.
- Participate in recruitment and training of care staff, ensuring a high standard of care is maintained at all times.
- Master's degree in Social Work, or a related field.
- Minimum of 2-5 years of experience in a managerial role in the healthcare industry (Acute, Ambulatory, or Post-Acute) and 2 years experience in Acute Care Management (Care Coordination).
- Willing to travel.
- Management experience is required.
- The position requires a California clinical Social Worker license.
- Knowledge of relevant regulations and best practices in the care sector.
- Strong leadership and communication skills.
- Excellent organizational and time-management abilities.
- Compassionate and empathetic approach to care.
- Ability to work well under pressure and make sound decisions.
- Proficient in Microsoft Office and other relevant software.
- Valid driver's license and access to a reliable vehicle.
