Role Summary:
As the Lead Case Manager, you will be an integral part of the Homeless Healthcare Collaborative team, focusing on enhancing health equity and improving clinical outcomes for individuals experiencing homelessness.

Key Responsibilities:
  • Deliver comprehensive care management by guiding clients without stable housing through the healthcare system and connecting them with resources that address their social health determinants.
  • Engage in outreach efforts and work collaboratively with a multidisciplinary street medicine team to identify, enroll, and provide case management services to eligible individuals for both medical and social support.
  • Partner with healthcare providers and community health workers to evaluate patient needs and deliver customized enhanced case management that addresses both clinical and social health requirements.
  • Formulate care plans and assist clients in accessing essential services, including medical, psychiatric, social, educational, housing, and nutritional support.
  • Travel to various community locations as necessary, collaborating with clinical teams, patients, caregivers, community partners, office personnel, and other vital stakeholders.
  • Promote awareness of UCLA Health's Homeless Healthcare Collaborative and ECM programs while working to improve the health outcomes of the unhoused population.

Compensation:
Salary range is $63,000 to $127,000 annually.

Qualifications:
  • Possession of a valid California Driver's License appropriate for the vehicle class operated, with no restrictions other than corrective lenses.
  • Current BLS certification from the American Heart Association or American Red Cross.
  • Demonstrated ability to interact with patients empathetically and diplomatically, while effectively communicating with staff, faculty, and the public.
  • Preferred experience in Whole-Person Care or Enhanced Care Management.
  • Experience in street medicine or community care for individuals experiencing homelessness is preferred.
  • Lived experience of homelessness is a plus.
  • Bilingual proficiency in English and Spanish is preferred.
