  • Lead Care Manager LCSW at our Health Center, located in Santa Monica, CA.
  • Collaborate with patients, caregivers, nurses, social workers, physicians, payers, support staff, and other practitioners to provide effective care coordination.
  • Facilitate communication and care coordination along a continuum through transitional care management.
  • Recognize the patient's right to self-determination and the significance of social determinants of health.

Job Qualifications:

  • Master's degree in social work from an accredited program.
  • California Clinical Social Worker License.
  • 2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute).
  • 2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.

Compensation: $94,972-150,321/year
