Role Responsibilities:
As the Lead Case Manager, you will be an integral part of the Homeless Healthcare Collaborative team, focusing on enhancing health equity and clinical outcomes for vulnerable populations.
Your primary responsibilities will include:
- Delivering comprehensive care management by guiding unhoused clients through the healthcare system and connecting them with essential resources that address their social determinants of health.
- Conducting outreach and collaborating with a multidisciplinary street medicine team to identify, enroll, and offer case management services to eligible individuals seeking medical and social support.
- Working closely with healthcare providers and community health workers to evaluate patient needs and deliver customized case management that addresses both clinical and social health requirements.
- Creating individualized care plans and facilitating access to medical, psychiatric, social, educational, housing, food, and other vital services.
- Traveling to various community locations as necessary, partnering with clinical teams, patients, caregivers, community organizations, office personnel, and other key stakeholders.
- Promoting awareness of UCLA Health's Enhanced Care Management and homeless healthcare initiatives while striving to improve the health outcomes of the unhoused population.
Compensation:
Salary range: $63,000 - $127,000 per year.
Qualifications:
- Valid California Driver's License appropriate for the vehicle class operated, with no restrictions other than corrective lenses.
- Ability to engage empathetically and diplomatically with patients across various health conditions and effectively communicate with staff, faculty, and the public.
- Preferred experience in Whole-Person Care or Enhanced Care Management.
- Experience in street medicine or community-based care for individuals experiencing homelessness is advantageous.
- Lived experience of homelessness is preferred.
- Bilingual proficiency in English and Spanish is preferred.
