Key Responsibilities:
- Collaborate with patients, caregivers, nurses, social workers, physicians, payers, and support staff to develop and implement effective care plans.
- Utilize case management principles to facilitate communication and care coordination along the continuum.
- Recognize the patient's right to self-determination and the significance of social determinants of health.
- Develop and maintain strong relationships with healthcare providers and stakeholders to ensure optimal patient outcomes.
Requirements:
- Bachelor's Degree in Nursing or equivalent education/experience.
- Master's Degree in Nursing or a related field is preferred.
- 2+ years of experience in healthcare, with a focus on case management or utilization management.
- California Registered Nurse License and current BLS certification.
Preferred Qualifications:
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification.
