Job Summary

Resiliency LLC is seeking a skilled Care Manager RN to join our team. As a Care Manager RN, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Collaborate with patients, nurses, social workers, physicians, and other healthcare professionals to develop and implement comprehensive care plans.
  • Conduct thorough assessments of patients' medical and social needs, identifying areas for improvement and developing strategies for optimal care.
  • Coordinate with healthcare providers to ensure timely and effective delivery of care, including medication management and follow-up appointments.
  • Provide education and support to patients and their families, empowering them to take an active role in their care.
  • Monitor patients' progress, making adjustments to care plans as needed to ensure optimal outcomes.
Requirements
  • Graduate of an accredited school of nursing.
  • California Registered Nurse License upon hire.
  • 3 years of clinical experience in an HMO, medical group, affiliated model, hospital, or medical office/clinic setting.
Preferred Qualifications
  • Bachelor's Degree in Nursing or a related field.
  • Master's Degree in a related field.
  • 3 years of experience in utilization management or case management.
