Job Title: Primary Care Practitioner

Meridian Health Services is seeking a highly skilled Primary Care Practitioner to join our team. As a Primary Care Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals and families in our community.

Responsibilities:

  • Provide direct nursing care to patients, including assessing their physical and mental health status, developing and implementing treatment plans, and evaluating patient outcomes.
  • Collaborate with treating physicians to ensure comprehensive care and make differential diagnoses of symptoms, functional problems, and risk behaviors.
  • Develop interventions to enhance patient health outcomes and maintain timely, complete, and accurate clinical and administrative records.
  • Partner with patients and their families to develop health plans and ensure a smooth transition between healthcare settings.
  • Perform all duties in accordance with Meridian Health Services standards, policies, and regulations.

Requirements:

  • Graduate of an accredited school of nursing with a master's degree in nursing from a Nurse Practitioner program.
  • Eligible for, or have prescriptive authority (DEA license) under the criteria outlined by the Indiana State Board of Nursing.
  • Current Registered Nurse (RN) licensure in the state of Indiana, or eligible for Indiana licensure.
  • Nurse Practitioner certification required (Family Practice, Psychiatric/Behavioral Health, and/or Pediatrics)
  • Two (2) years' related experience preferred
  • Valid driver's license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required
