Job Summary

Meridian Health Services is seeking a skilled Healthcare Professional to join our team as a Skill Development Specialist. In this role, you will provide skills and development training to assist patients and/or families in achieving developmental milestones and reducing psychosocial barriers.

Key Responsibilities:

  • Develop, implement, and revise individual treatment plans in collaboration with the clinical team.
  • Provide individual and group skills training and development to assist patients and/or families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving.
  • Assist patients and/or families in achieving developmental milestones and reducing psychosocial barriers.
  • Provide case management services as identified in the patient's treatment plan.
  • Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for the patient.

Requirements:

  • High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree.
  • Associate or Bachelor's degree preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
  • A commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.
