Job Overview

As a Youth Services Coordinator, you will play a vital role in supporting young individuals by collaborating with various service providers to create comprehensive service plans tailored to their unique needs.

Key Responsibilities:
  • Work collaboratively with the youth's primary service providers, including healthcare professionals and educational institutions, to formulate an Individualized Recovery Plan (IRP) that addresses their medical, behavioral, and social needs.
  • Conduct initial assessments and provide ongoing reports regarding the client and their family's circumstances.
  • Lead the implementation of the IRP, ensuring that all necessary services are coordinated effectively to support the youth's recovery journey.

Additionally, you will:

  • Identify and connect youth with essential services, making referrals as needed to support their recovery goals.
  • Assist youth in navigating between various services and supports to ensure they receive the necessary care.
  • Help families acquire resources such as vocational training, educational opportunities, and housing assistance.
Compliance and Documentation:
  • Adhere to all organizational policies regarding service authorization.
  • Participate in the development of service plans within the initial thirty days by gathering pertinent information.
  • Engage in planning and coordination efforts when clients face psychiatric hospitalization, incarceration, or homelessness.
  • Schedule and attend monthly supervision reviews and treatment team meetings.
  • Act as a liaison among families, referring agencies, and community organizations.
  • Ensure that clients receive appropriate and effective services aligned with their recovery objectives.
  • Maintain accurate case records and document all services provided.
  • Attend mandatory staff meetings and participate in discharge planning as required.
  • Uphold a high standard of confidentiality for all clients.
  • Collaborate effectively with other staff members assigned to cases.
  • Demonstrate professionalism in all interactions with clients and community partners.
  • Contribute to the development of crisis plans and assist in providing 24-hour crisis intervention as necessary.
  • Engage in quality improvement initiatives.
Additional Duties:

Other responsibilities may include:

  • Providing case management and psycho-social rehabilitation services.
  • Facilitating group training sessions for psychosocial skills.
  • Conducting family training and monitoring activities.
Work Environment:

At least 50% of service delivery must occur face-to-face with clients in non-clinic settings. The work schedule is flexible to accommodate the needs of clients, including availability on weekends and holidays.

Qualifications:

To be successful in this role, candidates must:

  • Maintain good standing with state licensure and professional liability.
  • Exhibit strong clinical writing and communication skills.
  • Have access to a secure computer for documentation purposes.
  • Possess excellent interpersonal skills and be a team player.
  • Demonstrate knowledge of strength-based and solution-focused treatment strategies.
  • Manage a caseload of clients with social behavior challenges.
  • Be self-motivated with a strong work ethic.
  • Hold a valid driver's license and have automobile insurance.
  • Pass a criminal background check and drug screening.
Training Requirements:

All staff in direct contact with clients must complete training in person-centered values, holistic care, and crisis intervention techniques within the first sixty days of employment.
