Job Summary

We are seeking a skilled and compassionate RN to join our team as a Nurse Care Manager. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with other healthcare professionals to achieve optimal patient outcomes.

Key Responsibilities
  • Assess, plan, and implement individualized patient care plans to meet the unique needs of each patient.
  • Collaborate with the Nurse Manager and other healthcare team members to ensure seamless care coordination and communication.
  • Use clinical judgment skills to diagnose and treat patient responses to actual or potential health conditions.
  • Develop and maintain accurate and comprehensive patient records, including documentation of nursing care and teaching.
  • Participate in ongoing process improvement activities to enhance patient care and outcomes.
  • Advocate for patient autonomy, dignity, and rights, while promoting a culture of respect and empathy.
  • Communicate effectively with patients, families, and healthcare team members to ensure continuity and coordination of care.
  • Initiate patient discharge planning and facilitate a smooth transition to the next point of service or home.
  • Identify, facilitate, and evaluate outcomes of nursing care for individual patients or groups of patients.
  • Provide evidence-based nursing care consistent with the plan of care, guidelines of care, physician orders, and health system policies and procedures.
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
  • Current RN licensure in the state of practice
  • Basic Life Support (BLS) certification
  • Advanced Cardiovascular Life Support (ACLS) certification within 90 days of hire
  • Pediatric Advanced Life Support (PALS) certification within 180 days of hire
  • Respiratory Therapist Registry (RRT) certification preferred
  • Ability to lift up to 6 inches from floor to waist
  • Ability to rotate trunk while standing and sitting
  • Visual acuity of 1.0 (20/20)
Work Environment

This role is a full-time position, working Monday to Friday with occasional weekend shifts. The ideal candidate will be able to work in a fast-paced environment and prioritize tasks effectively to meet the needs of patients and the healthcare team.
