About the Role

The RN Homecare Specialist will work under the direction of the Nurse Leader to provide professional nursing care to patients in their homes. This will involve assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities

  • Provide high-quality in-home care to patients, including assessment, planning, implementation, and evaluation of care
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Provide education and support to patients and their families on disease management, medication adherence, and other health-related topics
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication
  • Participate in ongoing education and training to stay current on best practices and industry developments

Requirements

  • Associate's degree in Nursing required; Bachelor's degree in Nursing preferred
  • Current RN licensure in Michigan required
  • At least one year of relevant experience in home health and/or other clinical setting required
  • Excellent communication and interpersonal skills required
  • Ability to work independently and as part of a team required
