Key Responsibilities- Assess, plan, and implement individualized patient care plans to meet the unique needs of each patient.
- Collaborate with the Nurse Manager and other healthcare team members to ensure seamless care coordination and communication.
- Use clinical judgment skills to diagnose and treat patient responses to actual or potential health conditions.
- Develop and maintain individualized patient plans of care, reflecting the full scope of practice and patient/family changing needs/goals.
- Deliver care in a culturally sensitive and patient-centered manner, preserving patient autonomy, dignity, and rights.
- Communicate effectively with patients, families, and healthcare team members to ensure continuity and coordination of care.
- Initiate patient discharge planning and facilitate a smooth transition to the next point of service/home.
- Identify, facilitate, and evaluate outcomes of nursing care for individual patients or groups of patients.
- Provide evidence-based nursing care consistent with the plan of care, guidelines of care, physician orders, and health system policies and procedures.
- Associate's Degree in Nursing (required)
- Bachelor's Degree in Nursing (required within 5-7 years, depending on start date and location)
- CRT-Adv Cardiovascular Life Support (ACLS) certification (within 90 days)
- CRT-Pediatric Adv Life Support (PALS) certification (within 180 days)
- CRT-Respiratory Therapist Registry (NBRC) certification (preferred)
This role is a full-time position, working Monday to Friday with occasional weekend shifts. The ideal candidate will be able to work in a fast-paced environment and prioritize tasks effectively.
