Position Title: Community Care Coordinator
Available Positions: Multiple openings
Reports To: Program Supervisor for Care Coordination
Job Purpose:
The Community Care Coordinator is responsible for managing care for individuals enrolled in the Care Coordination Program, focusing on those with HIV and/or mental health challenges.
In this role, you will actively foster an environment that aligns with the organization's Cultural Equity Policy and community engagement initiatives.
Key Responsibilities:
• Participate in bi-weekly team meetings for Care Coordination.
• Attend relevant training sessions and meetings pertaining to Care Coordination.
• Provide care coordination services as per the established model.
• Conduct New York State Community Mental Health assessments and follow-up evaluations.
• Process monthly billing for your assigned caseload.
• Collect and submit monthly performance data.
• Maintain electronic client records using designated software systems.
• Ensure client files are managed in accordance with program guidelines.
• Build and sustain relationships with community agencies and service providers.
• Represent the organization at various meetings as required.
Additional Responsibilities:
• Assist with answering the main office phone as needed to support team efforts.
• Engage in organizational committees as necessary.

Qualifications:
Education:
• A Bachelor's Degree in fields such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation therapy, rehabilitation, social work, sociology, or speech and hearing.
• Alternatively, a Bachelor's degree in any discipline with five years of direct experience working with individuals with behavioral health conditions.
• A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) is also acceptable.
Experience:
• A minimum of two years of relevant experience (a Master's degree in a related field may substitute for one year of experience).
• Experience in providing direct services to individuals with mental health or developmental disabilities, or in connecting them to essential community services.
• Bilingual proficiency in Spanish and English is preferred.
• Strong written and verbal communication skills are essential.
• Familiarity with Health Homes is highly desirable.
• Knowledge of Medicaid processes is preferred.
• Experience with Electronic Health Record systems is required.
• Proficiency in Microsoft Excel and Word is preferred.
• Ability to work independently with minimal supervision.
• Must be organized, self-driven, and capable of managing multiple tasks simultaneously.
• Ability to exercise sound judgment in crisis situations while adhering to confidentiality regulations.
• A valid Driver's License and a driving record that meets agency insurance standards are required.
Employment Terms and Benefits:
• This is a non-exempt position.
• Full-time commitment of forty (40) hours per week.
