Job Summary

StarCare Specialty Health System is seeking a qualified Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in ensuring that individuals with intellectual and developmental disabilities receive the support and services they need to live their best lives.

Responsibilities
  • Provide assessment, service planning, coordination, and monitoring of services for individuals on assigned caseload.
  • Conduct activities through face-to-face and telephone contacts with individuals, providers, family members/legal guardians.
  • Assist individuals on assigned caseload to establish and/or maintain eligibility for services.
  • Promote Person Centered Thinking and self-determination by educating individuals about rights, options, and choice.
  • Complete and submit all required program documentation in accordance with the Local IDD Authority (LIDDA) Department's designated timeframes.
  • Ensure all activities support and promote StarCare's culture of hope and health.
Requirements
  • Bachelor's degree from an accredited four-year college or university, required.
  • Major in a Human Services or related field (Sociology, Psychology, Family Studies, etc.), preferred.
  • One year of full-time experience working with individuals with intellectual or developmental disabilities, preferred.
