Role Overview:

As a Nutrition Consultant, you will deliver virtual medical nutritional therapy to Swap patients, leveraging your specialized expertise while adhering to our high standards of care and counseling. Our mission is to translate clinical nutrition guidelines into practical recommendations, offering personalized grocery shopping advice, food selections, products, and recipes tailored to each patient's nutritional requirements, lifestyle, and preferences.

At Swap, we prioritize exceptional patient care and seek dedicated nutrition professionals who share our commitment to fostering a safe and supportive environment. We aim to empower individuals to achieve and surpass their health and wellness objectives. You will be matched with patients and serve as their primary contact throughout their journey, conducting regular one-on-one sessions via phone or video conferencing.

Key Responsibilities:

  • Customized Patient Counseling: Deliver tailored nutritional guidance to patients using motivational interviewing techniques.
  • Enthusiasm for Nutrition: Exhibit expertise and comfort in discussing food recommendations, products, and their relevance to a patient's nutritional care, cultural practices, and lifestyle.
  • Operational Knowledge & Decision-Making: Demonstrate familiarity with the standard operating procedures and processes essential for the Nutrition Consultant role. Identify and address potential challenges using sound judgment and involve others in decision-making as necessary.
  • Skill Development: Show a commitment to enhancing your skills and competencies, whether independently or through team initiatives.
  • Quality of Care: Uphold the highest standards of patient care, striving to provide an exceptional patient experience.
  • Dependability & Productivity: Exhibit effective time management and ensure timely completion of assigned tasks while adhering to organizational schedules.
  • Effective Communication: Utilize strong verbal and written communication skills, remaining accessible to colleagues as needed.
  • Proactive Initiative: Display resourcefulness and teamwork, taking the initiative to address challenges in the care process and patient journey.
  • Patient-Centric Focus: Maintain high standards of patient satisfaction, actively seeking and responding to patient feedback while fostering long-term relationships.

Qualifications:

  • Bachelor of Science degree.
  • Active Registered Dietitian (RD) or Registered Dietitian-Nutritionist (RDN) credential from the Commission on Dietetic Registration.
  • Current licensure as a Dietitian in the state of Ohio.
  • Minimum of 3 years of experience in one-on-one patient counseling or coaching, with expertise in motivational interviewing.
  • At least 2 years of experience in a remote or virtual care setting.
  • Experience in discussing food products, label interpretation, and navigating online resources to make recommendations aligned with patient goals.
  • Familiarity with electronic health records for a minimum of 2 years.
  • Experience working with underserved populations for at least 1 year.
  • Experience managing patients with chronic conditions such as type 2 diabetes, chronic kidney disease, or heart disease for at least 2 years.
