About Communitas

Empowering individuals with intellectual and developmental disabilities, Communitas is dedicated to delivering individualized services, programs, and support. As a Day Program Case Manager at Communitas, you will play a vital role in fostering professional relationships with individuals and their interdisciplinary teams.

Estimated Salary: $22.00 per hour

Working in Wakefield, MA, Woburn, MA, or Beverly, MA, you will contribute to a positive impact in the lives of others by:

  • Ensuring that services meet individual needs and promote personal choices
  • Coordinating assessments and evaluations to determine support levels and planning
  • Maintaining follow-through and effectiveness of support plans with department staff and the Interdisciplinary team

Job Requirements

  • High School diploma required (BA degree in Human Services or related field preferred)
  • At least 1 year of experience supporting adults with developmental disabilities is preferred
  • A passion and dedication to supporting our people is a must
