About the Role
AHRC Nassau is seeking a highly skilled and dedicated Program Manager to lead our Day Habilitation Services team. As a key member of our organization, you will be responsible for developing and implementing individualized habilitation plans, coordinating activities, and supervising staff to ensure the best possible outcomes for our participants.
Key Responsibilities
- Develop and implement individualized habilitation plans in conjunction with program staff and ensure all required documentation is completed and maintained.
- Coordinate on-site and community-based activities for participants, including volunteer opportunities and community outings.
- Directly supervise Assistant Managers and Direct Support Professionals (DSPs) to ensure high-quality care and support.
- Oversee and participate in individual transports to and from the program, as well as to and from volunteer locations and other community-based activities.
- Maintain communication with parents/correspondents and service coordinators regarding participant progress, needs, and concerns.
- Provide direct support services to participants as needed, including assistance with daily living tasks and personal care.
- Ensure the protection of participants from neglect/abuse during program hours and report incidents to administration in a timely manner.
- Document all physical plant issues and ensure proper ordering and maintenance of furniture, equipment, and supplies.
- Attend meetings, conduct observations, and consult with staff to discuss participant progress, problems, and individual needs.
- Train and support staff to engage participants in tasks related to functional/meaningful activities and volunteer job assignments.
- Promote and reinforce AHRC Nassau's guiding principles and philosophies.
- Complete and maintain all required trainings/certifications determined by the needs of the program.
- Perform additional job-related duties as requested by supervisor.
Requirements
- High school diploma or equivalent required; Bachelor's degree in Human Services-related field preferred.
- Minimum one (1) year experience supporting people with intellectual and other developmental disabilities preferred.
- Good oral and written communication skills and computer proficiency in all Microsoft Office Suite applications preferred.
- Qualified NYS driver license and must be maintained during employment.
- AMAP/CPR/First Aid/SCIP-R and defensive driving training preferred.
Work Environment
The Program Manager will work in a fast-paced, dynamic environment with a team of dedicated professionals. The ideal candidate will be able to work independently and as part of a team, with excellent communication and problem-solving skills. The Program Manager will be required to lift and transfer participants, as well as transport them to and from the program and community-based activities.
