Job Summary:

The Senior Care Coordinator will be responsible for coordinating the delivery of healthcare services to our clients, ensuring their safety, comfort, and well-being. This includes managing a caseload of clients, coordinating with caregivers, and communicating with clients, families, and healthcare professionals.

Key Responsibilities:

  • Coordinate and manage the delivery of healthcare services to clients
  • Manage up to 15 clients and cover shifts for start-up clients and emergency shift coverage
  • Manage client expectations towards caregivers and serve as a liaison between the client, caregiver, and headquarters
  • Assist in client assessments and develop care plans
  • Train caregivers to ensure they are providing high-quality care
  • Support caregivers by providing support visits, training, advice, and correction as needed
  • Communicate with clients, families, and healthcare professionals to ensure continuity of care
  • Perform regular client check-ins and monitor their progress
  • Ensure compliance with healthcare regulations and agency policies
  • Travel to multiple clients as needed
  • Problem-solve in special customer care circumstances

Requirements:

  • Associates Degree or some college
  • Microsoft Office experience
  • At least 2 years of caregiving experience
  • CNA or HCA license
  • Nurse Delegation certificate
  • Valid Driver's License and willingness to travel to multiple clients in the same day
  • Must be okay with pets
