Responsibilities and Requirements

The Patient Care Facilitator will perform a variety of tasks to ensure smooth patient care operations, including:

  • Facilitating patient access to community resources and healthcare services
  • Developing and implementing care plans in collaboration with providers and staff
  • Conducting patient chart reviews to enhance visit efficiency and effectiveness
  • Participating in quality improvement initiatives and population health activities

To be successful in this role, you will need:

  • A minimum of 1-2 years of experience in a healthcare setting
  • Knowledge of medical terminology and computer applications
  • Strong communication and interpersonal skills
  • A high school diploma or equivalent

The estimated annual salary for this position is $55,000, based on location and industry standards.
