We are seeking a compassionate and skilled Registered Nurse to join our Transitional Care Unit team at Providence Little Company Of Mary Torrance Hospital. As a Transitional Care Nurse, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients and their families.
Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care plans.
- Collaborate with the interdisciplinary team to develop and implement individualized patient care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduation from an accredited nursing program.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 1 year Nursing experience.
