We are seeking a skilled Transitional Care Nurse to join our team at Providence Little Company Of Mary Torrance Hospital. As a Transitional Care Nurse, you will play a vital role in delivering high-quality patient care and ensuring a smooth transition for patients from hospital to home or other care settings.

Responsibilities
  • Provide comprehensive nursing care to patients with complex medical needs, including those with chronic conditions, disabilities, or terminal illnesses.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and make recommendations for interventions and referrals as needed.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Document patient progress, including medical history, treatment plans, and outcomes, in accordance with hospital policies and regulatory requirements.
Requirements
  • Graduation from an accredited nursing program.
  • California Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association upon hire.
  • 1 year Nursing experience.
