Responsibilities- Provide comprehensive nursing care to patients with complex medical needs, including those with chronic conditions, disabilities, or terminal illnesses.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and make recommendations for interventions and referrals as needed.
- Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
- Document patient progress, including medical history, treatment plans, and outcomes, in accordance with hospital policies and regulatory requirements.
- Graduation from an accredited nursing program.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 1 year Nursing experience.
