RN - Transitional Care Evening Shift - Full Time

We are seeking a skilled Transitional Care Registered Nurse to join our team at Providence Little Company Of Mary Torrance Hospital. As a member of our care team, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients and their families.
Responsibilities- Provide exceptional patient care and support to patients and their families in a transitional care setting.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
- Administer medications and treatments as prescribed by physicians and other healthcare professionals.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Graduation from an accredited nursing program.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 1 year Nursing experience.
