About the Role
We are seeking a skilled and compassionate Registered Nurse to join our Transitional Care team at Providence Little Company Of Mary Torrance Hospital. As a Transitional Care Nurse, you will play a vital role in providing high-quality patient care and supporting our patients' transition from hospital to home.
Key Responsibilities
- Provide direct patient care, including administering medications, performing treatments, and monitoring patient conditions.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and patient satisfaction.
Requirements
- Graduation from an accredited nursing program.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 1 year Nursing experience.
Preferred Qualifications
- National Certification in area of specialty upon hire.
