Care Manager RN FT/Evening

We are seeking a skilled Care Manager RN to join our team at Providence Little Company Of Mary Torrance Hospital. As a Care Manager RN, you will play a vital role in providing high-quality patient care and ensuring seamless transitions between healthcare services.
Key Responsibilities- Assess patient needs and develop individualized care plans to meet their comprehensive health needs.
- Collaborate with healthcare professionals to coordinate care and ensure continuity of services.
- Advocate for patients and their families to ensure their needs are met and their voices are heard.
- Provide education and support to patients and their families to promote health and wellness.
- Document patient information and progress in a timely and accurate manner.
- Bachelor's Degree in Nursing or related field.
- Current California Registered Nurse License.
- 2 years of acute hospital experience, including 1 year of supervisory experience.
- Strong knowledge base or experience in specialty area assigned.
- Master's Degree in Health Care related field.
