We are seeking a skilled Registered Nurse to join our Transitional Care team at Providence Little Company Of Mary Torrance Hospital. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.
Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of patient care.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduation from an accredited nursing program.
- California Registered Nurse License upon hire.
- National Provider BLS - American Heart Association upon hire.
- 1 year Nursing experience.
