RN - Transitional Care Evening Shift - Full Time

As a Transitional Care Nurse at Providence, you will play a vital role in providing high-quality patient care and support to patients transitioning from hospital to home or other care settings. This is a full-time, evening shift position, and you will work closely with a multidisciplinary team to ensure seamless care transitions.
Responsibilities- Provide comprehensive nursing care to patients with complex medical needs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare teams to ensure timely and effective care transitions
- Monitor patient progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Graduation from an accredited nursing program
- Current California Registered Nurse License
- National Provider BLS - American Heart Association
- 1 year of nursing experience
- National Certification in area of specialty
