Lung Transplant Nurse Coordinator Opportunity

Mass General Brigham is seeking a highly skilled and compassionate Lung Transplant Nurse Coordinator to join our team. As a key member of our transplant program, you will play a vital role in ensuring the seamless care of our patients throughout their transplant journey.

Key Responsibilities:
  • Coordinate patient care from pre-transplant to post-transplant, including scheduling appointments, testing, and follow-up care.
  • Collaborate with the transplant team, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care.
  • Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
  • Provide education and support to patients and their families, addressing their concerns and questions throughout the transplant process.
  • Manage and maintain accurate and up-to-date patient records, ensuring timely and effective communication with the healthcare team.
Requirements:
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • At least 2 years of experience in pulmonary, thoracic surgery, or transplant nursing, with a strong background in ambulatory care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Knowledge of community resources and post-acute care providers, with the ability to connect patients with necessary services.
