Nurse Manager Home Health

VNA Health Group is seeking a skilled Registered Nurse to fill a Home Health Manager position in Independence, Ohio.
This is a full-time opportunity for a compassionate and dependable individual to join our team and make a meaningful impact in healthcare.
As a Home Health RN Case Manager, you will be responsible for implementing therapeutic and rehabilitative nursing care, effectively communicating with healthcare providers and patients, and ensuring timely communication of relevant information to the healthcare team.
Key qualifications include experience in home health care, a valid Registered Nurse license in Ohio, and strong planning, organizational, and communication skills.

Responsibilities:
- Utilize the nursing process for data collection, planning, and evaluation of care.
- Implement therapeutic and rehabilitative nursing care with a focus on patient safety and comfort.
- Effectively communicate and collaborate with healthcare providers, patients, and families.
- Observe and document patient conditions and reactions, promptly notifying the appropriate healthcare team members.
- Respond to life-saving situations according to established protocols.
- Ensure timely communication of relevant information to the healthcare team.
Qualifications:
- Experience: Home health care experience preferred; 1-2 years in Med-Surg or hospital setting.
- Education: Accredited RN education program completion; Bachelor's degree in Nursing preferred.
- Licensure: Registered Nurse licensed in Ohio; valid driver's license and CPR certification required.
- Skills: Strong planning, organizational, and communication skills; ability to work under pressure and maintain confidentiality.
- Additional: Experience working with diverse communities; bilingual skills a plus.
