RN Case Manager Home Health Specialist

We are seeking a skilled and compassionate RN Case Manager to join our dynamic team at Visiting Nurse Association of Ohio. As a RN Case Manager, you will play a vital role in providing high-quality patient care and collaborating with healthcare professionals to achieve optimal outcomes.
Responsibilities- Utilize the nursing process to assess patient needs, develop care plans, and evaluate patient progress.
- Implement evidence-based practices to promote patient safety, comfort, and well-being.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Maintain accurate and detailed records of patient interactions, treatments, and outcomes.
- Participate in quality improvement initiatives to enhance patient care and services.
- 1-2 years of experience in Med Surg or hospital setting.
- Completion of an accredited RN education program with a diploma, Associates or Bachelor's degree. Bachelor's degree in Nursing preferred.
- Registered Nurse, licensed in the State of Ohio.
- Experience in home health care and 1 year experience in medical-surgical nursing. In addition to Acute Care experience.
- Valid driver's license and CPR certification is required.
