Job Summary:We are looking for a compassionate and experienced Registered Nurse to manage our Home Health program. The successful candidate will be responsible for providing high-quality patient care, leading a team of healthcare professionals, and ensuring the delivery of exceptional patient outcomes.
Responsibilities:- Provide direct and indirect patient care, including assessment, planning, implementation, and evaluation of care.
- Lead and manage a team of healthcare professionals, including RNs, LPNs, and CNAs.
- Develop and implement care plans that meet the unique needs of each patient.
- Collaborate with healthcare providers, families, and caregivers to ensure seamless care transitions.
- Monitor and evaluate patient outcomes, identifying areas for improvement and implementing changes as needed.
- 1-2 years of experience in home health care or a related field.
- Current RN licensure in the State of Ohio.
- Valid driver's license and CPR certification.
- Ability to work independently and as part of a team.
- Strong communication and leadership skills.
