Job OverviewWe are seeking a Patient Care Manager to join our team. As a Patient Care Manager, you will be responsible for the day-to-day operations of your assigned home health site, ensuring the provision of quality care to patients. You will work closely with referral sources, patients, caregivers, and employees to facilitate the relationship between them.
Key Responsibilities- Ensure that clients' plans of care are individualized and appropriate, executed as written, and reassessed by the appropriate healthcare professional when there is a significant health status change in the client's condition.
- Ensure appropriate documentation is completed for all patients transferred to an inpatient facility.
- Manage the site in the absence of the Executive Director or Administrator.
- Oversee clinical and administrative and recordkeeping functions.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
- Associates degree in Nursing from an accredited school of nursing.
- Bachelor degree in nursing from an accredited school of nursing preferred.
- Minimum three years experience in direct patient care in a clinical setting.
- Must be able to effectively operate nursing assessment tools and office equipment, including computers and tablets.
- Current RN licensure in the state of practice.
- Current CPR certification.
