Job Title: Home Health RN Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to fill the role of Home Health RN Manager. This position is responsible for providing leadership and oversight to a team of healthcare professionals, ensuring the delivery of high-quality patient care in the comfort of their own homes.

Responsibilities:
  • Lead and manage a team of RNs and other healthcare professionals to provide comprehensive care to patients in their homes.
  • Develop and implement care plans that meet the unique needs of each patient, in collaboration with the interdisciplinary team.
  • Conduct regular assessments and evaluations to ensure the quality and effectiveness of care provided.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Ensure compliance with regulatory requirements, including HIPAA and OSHA standards.
  • Develop and maintain relationships with community partners, including hospitals, skilled nursing facilities, and other healthcare organizations.
Requirements:
  • Current RN licensure in the state of Ohio.
  • Minimum of 2 years of experience in home health care, with a focus on leadership and management.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with a focus on collaboration and problem-solving.
  • Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
