Job SummaryWe are seeking a Patient Care Manager to join our team. As a key member of our leadership team, you will be responsible for the day-to-day operations of our home health sites, ensuring the provision of quality care to patients. You will work closely with referral sources, patients, caregivers, and employees to facilitate the delivery of care.
Key Responsibilities- Develop and implement individualized plans of care for patients, ensuring they receive the highest quality care.
- Manage the clinical and administrative aspects of our home health program, including documentation and record-keeping.
- Supervise and mentor nursing staff, providing guidance and support to ensure excellent patient care.
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes.
- Collaborate with other departments to ensure seamless care coordination and communication.
- Associate's degree in Nursing from an accredited school of nursing.
- Minimum three years of experience in direct patient care in a clinical setting.
- Current RN licensure in the state of practice.
- Current CPR certification.
