About the Role

We are seeking a highly skilled and experienced Patient Care Manager to join our team at Heartland Home Health. As a Patient Care Manager, you will play a critical role in ensuring the provision of high-quality care to our patients and their families.

Key Responsibilities
  • Operational Management: You will be responsible for the day-to-day operations of your assigned home health site, including administering the clinical aspects of the home health program.
  • Clinical Oversight: You will ensure that the clients plan of care is individualized and appropriate, executed as written and reassessed by the appropriate healthcare professional when there is a significant health status change in the client's condition.
  • Documentation and Recordkeeping: You will ensure that all necessary documentation is completed for all patients transferred to an inpatient facility.
  • Leadership and Management: You will manage and oversee clinical and administrative functions, including managing the site in the absence of the Executive Director or Administrator.
  • Collaboration and Communication: You will participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.
Requirements
  • Education: Associate's degree in Nursing from an accredited school of nursing. Bachelor's degree in nursing from an accredited school of nursing is preferred.
  • Experience: Minimum three years of experience in direct patient care in a clinical setting.
  • Licensure and Certification: Current RN licensure in the state of practice and current CPR certification.
  • Skills and Abilities: Ability to effectively operate nursing assessment tools and office equipment, including computers and tablets.
