About the Job
St. David's Hospice & Family Care is seeking an experienced and empathetic Hospice Care Coordinator to join our care team. This role involves coordinating hospice services, collaborating with interdisciplinary teams, and ensuring seamless patient care transitions.
Responsibilities:- Coordinate hospice services, including scheduling appointments, managing referrals, and maintaining accurate records.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure comprehensive care.
- Bachelor's Degree in Nursing or related field required.
- Basic Cardiac Life Support certification preferred.
- Minimum 1 year of experience in hospice care or a related field.

Compensation:$60,000 - $75,000 per year, based on experience.
