About the Role

We are seeking a compassionate and dedicated RN Case Manager to join our team at Bristol Hospice. As a key member of our care delivery team, you will play a vital role in providing exceptional patient-centered end-of-life care.

Job Overview

This is a full-time position with a salary range of $80,000 - $110,000 per year, depending on experience. The ideal candidate will have a strong nursing background, excellent communication skills, and a passion for delivering high-quality care.

Responsibilities
  • Create and initiate care plans through assessing the history of current and previous illnesses.
  • Provide quality one-on-one time with patients and their families to support everyday needs.
  • Care for patients by providing initial and ongoing assessments of patient and family needs.
  • Record important health information, including temperature, pulse, respiration, and blood pressure.
  • Administer medications and treatments as prescribed by the physician.
  • Provide healthcare instructions to the patient as appropriate.
  • Record symptom management changes and outcomes.
  • Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given.
Requirements
  • An enthusiasm for providing the best possible care to patients and their families.
  • An RN license in good standing.
  • 1 year of experience in hospice or home health preferred.
  • A valid driver's license.
  • Current CPR certification.
  • Excellent observation, verbal, and written communication skills.
