Clinical Care Coordinator

Salary and BenefitsWe offer a competitive salary, ranging from $65,000 to over $90,000 annually, depending on experience. Additionally, you can expect:

The Registered Nurse (RN) will provide care and services for a group of hospice patients, visiting them in their homes, assisted living facilities, or hospitals. This role requires working seven days/nights on and seven nights/days off. As a Clinical Care Coordinator, you will be responsible for:
- Providing individualized care and support to patients and their families
- Collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership
- Managing patient visits, documentation, meetings, and communication
To succeed in this role, you will need:
- A current Registered Nurse (RN) licensure in good standing, with one year of experience under license
- Admissions and on-call experience, preferably
- Strong time management skills, ability to prioritize tasks, and triage multiple concerns
- Reliable transportation, valid driver's license, and auto insurance
