Position Overview:

This role is pivotal in achieving key performance outcomes and system enhancement objectives, including financial, satisfaction, and clinical metrics, as part of the nursing component within the care coordination framework. The incumbent will enhance patient outcomes by minimizing hospital readmissions and facilitating care episodes for patients within specific populations or disease categories. Responsibilities include identifying suitable patients, fostering patient and family involvement in self-care management, ensuring smooth transitions to subsequent care levels through effective communication, conducting educational sessions for patients and families using the 'teach-back' method, and aiding patients in navigating the healthcare landscape.

Key Responsibilities:

  • Oversee the assessment process for the designated patient demographic, documenting necessary information in collaboration with team members.
  • Enhance patient access by acting as a liaison between referring providers and Vanderbilt healthcare professionals.
  • Engage routinely with the clinical team to formulate a collaborative care plan from the initial hospitalization through the defined care continuum.
  • Develop and manage processes related to pre-admission and post-discharge transitions, establishing clinical pathways with providers to optimize care.
  • Assist in creating and distributing patient education materials, including tailored medication grids with input from the pharmacy team.
  • Collaborate with multidisciplinary teams for discharge and transition planning, actively involving outpatient care providers and sharing pertinent information.
  • Exhibit reflective practice by continually assessing care coordination and supporting the development of evidence-based protocols.
  • Additional responsibilities may be assigned as needed.
