Position Summary:
The Care Coordinator RN plays a crucial role in the oncology care continuum, utilizing critical thinking and nursing processes to assess and address the needs of patients and their caregivers. This position involves monitoring patient status and treatment plans while facilitating communication among patients, healthcare providers, and the care team.
Key Responsibilities:
1. Coordinate the timely scheduling of appointments, diagnostic tests, and treatment plans to streamline patient care.
2. Ensure that essential medical records are available during appointments.
3. Maintain regular communication with patients and healthcare providers to evaluate health status and satisfaction with care.
4. Track patient progress and collaborate with care providers to eliminate barriers to treatment.
5. Educate patients about their disease processes, treatment options, and wellness strategies, including diet and exercise.
6. Connect with patients before and after significant treatments or hospital visits to assess needs and understanding of care plans.
7. Facilitate referrals to ancillary services and ensure timely follow-through on appointments.
8. Prepare and submit reports related to care coordination performance and metrics.
Qualifications:
- Current RN state license.
- Minimum of 1 year of experience in oncology nursing or relevant care coordination.
- Certification in Oncology Nursing required within 15 months of hire.
- Current ONS Chemotherapy/Biotherapy Card required within 6 months of hire.
- AHA/ARC BLS certification required within 90 days of hire for on-site roles.
Physical Requirements:
Must be able to lift up to 50 pounds and travel to assigned clinics as needed.
