About the Role:

MidState Medical Center is seeking a highly skilled and compassionate Clinical Coordinator to join our team. As a key member of our Outpatient Spine and Pain center, you will play a vital role in delivering exceptional patient care and ensuring a seamless experience for our patients.

Key Responsibilities:

  • Assess patient conditions, set goals, and develop individualized care plans to meet their physical, psychological, social, and rehabilitative needs.
  • Coordinate nursing care for patients undergoing therapy or procedures, ensuring timely and effective communication with other departments.
  • Establish and maintain open communication with patients, families, and healthcare teams, utilizing teaching opportunities to promote patient education and empowerment.
  • Maintain high standards of nursing care, adhering to hospital policies and procedures, and prioritizing tasks to ensure efficient and effective care delivery.
  • Collaborate with physicians and other healthcare professionals to provide comprehensive care and address patient needs.

Requirements:

  • Associate Degree in Nursing required
  • Valid CT RN license
  • CPR and ACLS certification
  • Excellent written, verbal, and communication skills
