Community Habilitation Specialist

We are seeking a highly motivated and compassionate Community Habilitation Specialist to join our team at Upstate Caring Partners. As a key member of our Community Services team, you will play a vital role in supporting individuals with developmental disabilities to live fulfilling lives.
Key Responsibilities- Conduct initial waiver service intakes with individuals and their families to identify habilitation needs and develop personalized support plans.
- Develop and monitor individualized Staff Action Plans to ensure effective delivery of habilitation services.
- Promote independence and person-centered programming to support individuals in achieving their goals.
- Coordinate group activities and facilitate community engagement to enhance social connections and community integration.
- Deliver habilitation services to individuals, assessing the appropriateness of plans and making adjustments as needed.
- Supervise direct support staff, providing performance feedback, training, and coordination of staffing hours.
- Maintain accurate and up-to-date documentation, ensuring compliance with regulatory requirements.
- Participate in leadership activities, including meetings, trainings, and committee involvement, to contribute to the growth and development of our team.
- Associate's Degree in human services or a related field (e.g., psychology, sociology).
- Minimum of 12 months of experience working with individuals with developmental disabilities.
- A minimum of 18 hours per week of direct service delivery is required.
- Valid NYS driver's license and reliable transportation.
