Job Summary

We are seeking a highly motivated and compassionate Case Manager to join our team at RCIL. As a Case Manager, you will play a critical role in supporting youth with disabilities who are at risk due to various challenges.

Key Responsibilities
  • Conduct Risk Assessments: Identify potential crisis situations and develop strategies to mitigate risks.
  • Develop Support Service Plans: Create personalized plans to address the unique needs of each youth.
  • Maintain Regular Contact: Build strong relationships with youth and their families through regular visits and communication.
  • Manage Case Files: Maintain accurate and up-to-date records of each youth's progress and services provided.
  • Provide Information and Referral Services: Connect youth and families with community-based services and resources.
Requirements
  • Experience Working with Youth: Proven experience working with youth with disabilities, either professionally or personally.
  • Strong Advocacy and Networking Skills: Ability to build strong relationships with youth, families, and community partners.
  • Computer Skills: Proficiency in Microsoft Office and other relevant software.
  • Knowledge of Community-Based Services: Familiarity with local resources and services that support youth with disabilities.
  • Excellent Customer Service Skills: Ability to provide exceptional service to youth, families, and community partners.
Education and Experience
  • Associate's Degree or Equivalent: Preference given to candidates with an Associate's degree or higher in a related field.
  • Experience May Be Considered: Relevant experience may be considered in lieu of higher education.
