We are seeking a highly motivated and compassionate Community Habilitation Specialist to join our team at Upstate Caring Partners. As a key member of our Community Services team, you will be responsible for providing individualized support and services to individuals with developmental disabilities, promoting their independence and well-being.
Key Responsibilities- Conduct initial assessments and develop personalized support plans to meet the unique needs of each individual.
- Provide direct support and services to individuals, including habilitation, training, and skill-building activities.
- Collaborate with families, caregivers, and other stakeholders to ensure seamless support and services.
- Monitor and evaluate the effectiveness of support plans, making adjustments as needed to ensure optimal outcomes.
- Supervise and train direct support staff, ensuring they have the necessary skills and knowledge to provide high-quality support.
- Maintain accurate and up-to-date records, ensuring compliance with regulatory requirements.
- Associate's degree in human services or a related field.
- Minimum 12 months of experience working with individuals with developmental disabilities.
- Valid NYS driver's license and reliable transportation.
