Patient Care Coordinator

Healogics, Inc. is seeking a highly skilled Patient Navigator to join our team. As a Patient Navigator, you will play a critical role in ensuring the smooth operation of our outpatient wound care centers.
Key Responsibilities:- Coordinate with center leadership to maximize daily patient census.
- Participate in staff meetings to support key functions within the center.
- Greet patients and other visitors, answer and route calls to appropriate staff.
- Maintain all patient communication needs, including scheduling, rescheduling, and appointment reminders.
- Obtain and verify patient insurance information, including pre-certifications and pre-authorizations for services.
- Collect and enter patient charges in databases.
- Verify and reconcile charges as directed, prepare monthly patient survey data, and transmit to providers.
- Assemble new patient charts, maintain and file existing patient charts, and spot check charts for data completeness and signatures.
- Coordinate/schedule ancillary testing with other hospital departments.
- Arrange for patient transportation as needed.
- Maintain office equipment and supplies as needed, and medical supplies as directed.
- High School Diploma or General Education Development (GED); Associate's degree preferred.
- Minimum of Two (2) or more years office administration experience, preferably in a medical setting; prior medical coding experience preferred.
- Proficient in Microsoft Office (Word, Excel, Outlook).
- Good customer, interpersonal, and communication skills, both orally and in writing.
- Organization and time-management skills.
- Ability to type 60 words per minute (wpm).
- Basic math skills.
- Attention to details.
- Ability to maintain confidentiality.
- Ability to work in a fast-paced environment and to work on multiple projects at the same time.
- Ability to work with others and in a team environment.
The hourly rate for this position generally ranges between $21.81-$28.43 Per Hour. This range is an estimate, based on potential employee qualifications: education, experience, geography, as well as operational needs and other considerations permitted by law.
