Job Title: Physical Therapist Home Health

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their own homes, helping them to improve mobility, reduce pain, and restore function.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement personalized treatment plans in consultation with physicians and revise them as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as they relate to physical therapy.
  • Assist physicians in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests, recording and reporting any adverse reactions to treatment.
  • Treat clients to relieve pain, develop or restore functional ability, and maintain maximum performance using physical means such as exercise, massage, heat, light, and electricity.
  • Teach, supervise, and counsel family members and clients in the physical therapy program, instructing them in the care and use of adaptive devices such as wheelchairs, braces, crutches, canes, prosthetic, and orthotic devices.
  • Record all evaluation data, treatments, and clients' responses to therapy intervention on a timely basis, as well as quality assurance data as directed.
  • Prepare evaluations of clients' conditions and perform periodic reassessments.
  • Supervise Physical Therapist Assistants at least every 30 days.
  • Participate in case conferences, physical therapy staff meetings, agency activities, and in-service programs as necessary, participating with staff, clients, and physicians in discharge planning activities and completing a Physical Therapy Discharge Summary.
  • Supervise home health aides every 14 days when physical therapy is the only professional discipline involved in the client's care.
  • Evaluate clients, their significant others, and the home situation to determine what instruction will be required, what assistance will be available from the family in caring for the client, and what other agency and community services will be required.
Requirements:
  • The ability to drive and access clients' homes, which may not be routinely wheelchair accessible, is required.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client's condition and to perform client care.
  • On occasion, may be required to bend, stoop, reach, and move client weight up to 250 pounds, lift and/or carry up to 30 pounds.
  • Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Must be licensed or registered by the State.
  • Must have at least one year of institutional experience.
  • The Physical Therapist in charge of PT services must have a home care experience and at least one year of supervisory experience (2 years preferred).
  • Must have one year of experience in an acute/sub-acute setting.
Education and Certifications:
  • Doctorate or better.
  • BLS.
  • PT.
