Job Summary

We are seeking a skilled and compassionate Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Pinnacle Home Care Inc., you will be responsible for providing physical therapy services to patients in their homes, assessing their conditions, developing personalized treatment plans, and delivering therapy to improve mobility, reduce pain, and restore function.

Responsibilities
  • Assess patients' conditions and develop personalized treatment plans
  • Deliver physical therapy services to patients in their homes
  • Travel to patients' homes and adapt treatment plans to suit home environments
  • Ensure high-quality care and patient safety
  • Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
  • Treat clients to relieve pain, develop or restore functional and maintain maximum performance, using physical means, such as exercise, massage, heat, light and electricity
  • Teach, supervise, and counsel the family and client in the physical therapy program
  • Instruct client in care and use of adaptive devices such as wheelchairs, braces, crutches, canes and prosthetic and orthotic devices
  • Record on a timely basis all evaluation data, treatments, and client's response to therapy intervention
  • Participate in case conferences, physical therapy staff meetings, agency activities and in-service programs as necessary
  • Participate with staff, clients, and physicians in discharge planning activities and completes a Physical Therapy Discharge Summary
  • Supervise the home health aide every 14 days when physical therapy is the only professional discipline involved in the client's care
Requirements
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A.
  • At least one year of supervisory experience
  • Home care experience
